About The Position
Mountain Community Health Partnership (MCHP) is seeking a full time Registered Nurse (RN) Care Manager. The RN Care Manager plays a vital role in coordinating comprehensive care for patients across the continuum of health services. This position involves assessing, planning, implementing, and monitoring care plans to achieve quality outcomes. Works collaboratively with providers, patients, caregivers, and community resources to ensure care is patient focused.
Requirements
- Bachelor's degree in nursing; current RN license in NC required.
- Minimum 5 years in ambulatory care management, acute care, home health, or public health.
- Strong communication and organizational skills.
- Commitment to confidentiality and patient-centered care.
Nice To Haves
- Certified Case Manager (CCM) preferred.
Responsibilities
- Conduct comprehensive assessments addressing medical, psychosocial, behavioral, and cultural needs.
- Develop and implement individualized care plans in partnership with patients and care teams.
- Identify barriers to care and implement interventions to improve health outcomes.
- Provide patient education on diagnoses, medications, and self-management strategies.
- Coordinate referrals and serve as a liaison among patients, providers, and community resources.
- Monitor progress toward patient-centered SMART goals and adjust plans as needed.
- Support organizational performance improvement initiatives and data analysis.
