Requirements

Possess a valid NYS RN License
Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
An Associate's degree or higher
Demonstrate a passionate attitude
Exceptional communication skills
Strong problem-solving abilities
Bilingual proficiency is essential, with a particular focus on languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
Ability to effectively navigate and utilize various software and tools relevant to care management

Responsibilities

Play a crucial role in all aspects of care management, including care planning, coordination, and health assessments
Monitor the quality and effectiveness of services
Track progress towards individual goals
Ensure the highest level of care for our patients
Oversee transitions for patients
Facilitate smooth discharge planning from hospitals or nursing homes
Identify health, environmental, and psychosocial risks
Recommend and implement interventions in collaboration with the Interdisciplinary Care team
