About The Position

Community Care is hiring a Registered Nurse/RN Case Manager to join our Family Care Program team. This is a full-time opportunity working out of our Sheboygan County location. Sign On Bonus Available, $8,000

Requirements

• Education: Associate’s Degree in Nursing Required, Bachelor’s Degree preferred.
• Licensure: Wisconsin Licensed Registered Nurse
• Licensure: Eligibility for long-term care functional screener certification is required
• Licensure: WI Driver’s License
• Experience: Required 2 years’ recent experience with older adults or individuals with physical or intellectual disabilities in a nursing capacity

Nice To Haves

• Preferred 2 years of work in community nursing

Responsibilities

• Completes discipline-specific initial and ongoing assessments.
• Conducts home visits and home assessments as needed and consults w/ families and other supports for additional information as needed.
• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports.
• Follow up on all services to ensure quality/coordination and effectiveness of services.
