CommWell Health is seeking a compassionate, organized, and clinically strong Registered Nurse Care Manager who thrives in a mission-driven environment and is passionate about coordinating care for patients living with multiple chronic conditions . In this role, you’ll work as part of a multidisciplinary team focused on population management , helping patients improve outcomes through education, care planning, and strong transitions of care support. Requirements

  • Graduate of an accredited RN program
  • Current North Carolina RN License
  • 2+ years of experience in primary healthcare, ambulatory care and/or home health within the last 5 years
  • HS Diploma or GED
  • Comfortable navigating an electronic health record (EHR)
  • Valid NC driver’s license + clean driving record (approved for driving privileges)

Nice To Haves

  • Experience with underserved or rural populations
  • Spanish speaking preferred (not required)

Responsibilities

  • Identifying patients who meet criteria for the care management program
  • Reviewing charts to assign acuity levels and assign patients to the appropriate care team
  • Providing oversight to Care Coordinators completing comprehensive assessments and assisting with development of individualized Care Plans
  • Developing and updating care plans in collaboration with the patient, PCP, and care team
  • Supporting transitions of care, including medication reconciliation, follow-up, specialist referrals, barriers to care, and education
  • Coordinating communication with community partners such as hospitals, emergency departments, and discharge planners
  • Supporting PCMH implementation and helping teams understand care coordination workflows
  • Monitoring outcomes, identifying gaps, and recommending improvements
