About The Position

The Remote RN Care Manager plays a key role in ensuring high-quality, patient-centered end-of-life and palliative care. This position provides telephonic and virtual support to patients and their families while collaborating with physicians, nurse practitioners, and interdisciplinary team members to develop and implement individualized care plans. As a RN Care Manager in our Palliative Care Advanced Illness Management Program, You Will: + Conduct virtual or telephonic RN screening assessments and provide education to AIM patients, family members, and caregivers. + Complete risk stratification assessments to identify patient needs according to established protocols. + Maintain consistent outreach frequency per patient needs and risk level. + Deliver reliable education on disease processes, symptom management, self-care, nutrition, and medication administration. + Collaborate with the AIM nurse practitioner, physician, and interdisciplinary team to coordinate care and update the plan of care as needed. + Communicate any changes in patient condition, signs of decline, or new care needs promptly to the appropriate team members. + Document all assessments, findings, interventions, and care plans accurately and in a timely manner. + Support hospice referrals and transitions of care as appropriate. + Facilitate prescription requests, lab results, and durable medical equipment needs per clinical guidelines. + Provide education to patients and families that promotes comfort, confidence, and quality of life. + Participate in after-hours or weekend call coverage as assigned. + Adhere to all company, HIPAA, and compliance policies while maintaining confidentiality and professionalism in every interaction. Schedule: Monday-Friday, 8am-5pm Coverage: Toledo, OH Palliative Care Branch - Must reside and have licensure in Ohio

Requirements

  • Minimum of 2 years of prior skilled Registered Nurse experience
  • Demonstrated ability to provide high-quality patient care
  • Active, unencumbered Registered Nurse license in state(s) of practice
  • Fluent competence in Microsoft Outlook, Word, and Excel
  • Proficiency in point-of-care and electronic medical record systems
  • Excellent leadership, interpersonal, written, and verbal communication skills
  • Strong independent decision-making and self-motivation
  • Working knowledge or understanding of the principles of Palliative Medicine, including patient appropriateness for home health or hospice care
  • Understanding of Medicare/Medicaid regulations and the interdisciplinary process
  • Meets mandatory continuing education requirements of Gentiva and state licensing board
  • Prior clinical experience with strong problem-solving and time management skills
  • Compassionate communicator with patients and families
  • Keen observation skills and ability to identify and escalate changes in patient conditions appropriately

Nice To Haves

  • Experience in palliative care, oncology, hospice, advanced illness management, or geriatric care preferred
