About The Position

This Care Manager position is full-time and requires working onsite at Piedmont Fayette, Monday through Friday from 8:00 AM to 4:30 PM. The primary responsibilities of the Care Manager include comprehensive care coordination, ensuring the progression of care for patients, and engaging in proactive discharge planning. A critical aspect of this role is to expedite the timely and safe discharge for all patients within their assigned caseload.

Requirements

• Graduate of Nursing Program
• 2 years of experience in an acute or post-acute setting
• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure

Nice To Haves

• Master’s degree
• Experience using InterQual or Milliman
• Certification in Case or Care Management
