About The Position

Community Care is offering a $8,000 Sign-on Bonus for Full-Time RN Care/Case Manager to join the Day Shift. This is a full-time opportunity for Racine County. RN Care/Case Manager Job Responsibilities: Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs. Conduct phone and in-person assessments using decision tools and resources to identify immediate and potential health needs. Complete all required documentation and charting and assists in the development and maintenance of member records Job Requirements: Education: Associate degree in nursing required. Experience: Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred. Licensure: A current Wisconsin Registered Nurse license in good standing RN Care/Case Manager Job Schedule: Work Schedule: Monday through Friday (No call, no weekends) Hours of Operation: 8:00 a.m. - 4:30 p.m. (40 hours per week) About Community Care: Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. Requirements

  • Associate degree in nursing required.
