About The Position

The position is part-time, requiring 24 hours of work per week, specifically every weekend from 7am to 7pm, with occasional holidays. The role involves utilizing the RN process to focus healthcare team activities on achieving optimal outcomes, resource utilization, clinical expertise, and improvement strategies. The nurse will interact with patients, professionals, and the community to ensure continuity of care and coordination of services, documenting plans of care across multiple settings. Responsibilities include conducting comprehensive needs assessments for patients, developing and maintaining accurate case records, and ensuring knowledge of reimbursement criteria for services provided.

Requirements

• BSN required; Master's preferred.
• Current RN license or temporary license as a Registered Nurse.
• Two years' experience in healthcare, including case management/discharge planning.
• Appropriate professional certification required within 3 years of hire.
• Clinical competence in disease management and case management principles.
• Excellent interpersonal communication and negotiation skills.
• Strong organizational and time management skills.
• BLS required per policy guidelines.

Responsibilities

• Utilizes RN process as a framework for healthcare team activities.
• Interacts with patients and professionals to achieve continuity of care.
