About The Position

"Navitas Healthcare, LLC" is seeking an experienced RN Care Manager – Emergency Department to support care coordination, utilization review, and discharge planning within a fast-paced ED environment. This role partners closely with physicians, nursing staff, and interdisciplinary teams to ensure safe, timely, and effective patient transitions across care settings while maintaining regulatory compliance and high-quality patient-centered care.

Requirements

• Minimum 5 years of clinical experience as a Registered Nurse
• Experience in care coordination, utilization review, and discharge planning
• Strong computer literacy with demonstrated EMR proficiency
• Active Registered Nurse (RN) license in the State of New York
• Basic Life Support (BLS) certification
• PRI and Screen certification (must be obtained within three months of employment)
• Primary Source Verification of RN license through the New York State Department of Education (NYSED)
• Associate Degree in Nursing (ASN or ADN) from an accredited nursing program

Nice To Haves

• Prior leadership experience
• Case management or care management experience
• Bachelor’s or Master’s degree in Nursing

Responsibilities

• Coordinate patient care and discharge planning within the Emergency Department
• Perform utilization review to ensure appropriate levels of care and efficient resource utilization
• Collaborate with physicians, nursing staff, and interdisciplinary care teams
• Facilitate timely and safe transitions of care across inpatient, outpatient, and community settings
• Ensure compliance with documentation, regulatory, and accreditation requirements
• Utilize electronic medical records (EMR) and clinical systems to support care coordination
• Educate patients and families regarding discharge plans, services, and follow-up care
• Participate in quality improvement initiatives and care management activities
• Demonstrate leadership, professionalism, and a patient-centered approach to care
