About The Position

The position is for a full-time Registered Nurse (RN) in Case Management at Atrium Health Cabarrus. The role involves utilizing the RN process to focus healthcare team activities on achieving optimal outcomes, resource utilization, and improvement strategies. The RN will interact with patients, professionals, and the community to ensure continuity of care and coordination of services. Responsibilities include conducting comprehensive needs assessments, developing and maintaining accurate case records, and demonstrating leadership in nursing practice. The RN must also maintain knowledge of reimbursement criteria and documentation necessary for reimbursement, including Medicaid, Medicare, and Managed Care.

Requirements

  • BSN required; Master's preferred.
  • Current RN license or temporary license as a Registered Nurse.
  • Two years' experience in healthcare, including case management/discharge planning.
  • Appropriate professional certification required within 3 years of hire.
  • Clinical competence in disease management and case management principles.
  • Excellent interpersonal communication and negotiation skills.
  • Strong organizational and time management skills.
  • Demonstrated knowledge and proficiency in appropriate tools.
  • BLS required per policy guidelines.

Responsibilities

  • Utilizes RN process as a framework for healthcare team activities.
