About The Position

The Care Manager facilitates a lifelong, proactive partnership with patients to enhance and personalize management of health-related needs. The role involves assessing needs, planning, coordinating, and evaluating services for patients, aiming to empower individuals and their families to access resources and adopt healthy lifestyles. Care Managers focus on five primary areas: population management, patient self-management support, transitions in care, resource connection, and appropriate resource utilization. They work collaboratively with a multidisciplinary care team to achieve timely and appropriate patient management.

Requirements

  • BSN required; Master's preferred.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state of work.
  • Two years' experience in healthcare, including case management/discharge planning.
  • Appropriate professional certification required within 3 years of hire.
  • Clinical competence in disease management and case management principles.
  • Excellent interpersonal communication and negotiation skills.
  • Strong organizational and time management skills.
  • Ability to work independently and as part of a care team.
  • BLS required per policy guidelines.

Responsibilities

  • Utilizes RN process as a framework to focus healthcare team activities on optimal outcomes and resource utilization.
  • Interacts with patients, professionals, and the community to achieve continuity of care and coordination of services.
  • Conducts comprehensive needs assessments for identified patients and matches care-related services to their needs.
  • Develops and maintains accurate case records for each referred customer/patient.
  • Documents care plans fully and accurately using appropriate computer databases and documentation systems.
  • Maintains knowledge of reimbursement criteria and documentation necessary for reimbursement.
  • Demonstrates leadership in nursing practice and evaluates nursing practice against professional standards.
