About The Position

Stay Amazing RN Care Management Coordinator - Pediatric Complex Care Management – Full-Time – Ambulatory Care Network- NewYork Presbyterian Hospital/Weill Cornell. Step into NewYork-Presbyterian Hospital/Weill Cornell Ambulatory Care Network (ACN) and help drive the success of our Pediatric Care Management Program. Be a part of the team that transforms healthcare delivery to vulnerable patients with complex medical needs. As a Nurse Care Manager within our ACN, you will be responsible for providing intensive care management for pediatric patients with complex medical needs. You will develop individualized care plans, and collaborate with an interdisciplinary team of physicians, advanced practitioners, social workers and other stakeholders, to provide a comprehensive approach to care. Your efforts & plans will help to minimize and prevent hospital readmissions throughout our health system for patients of special needs. Through your expertise and knowledge, you will help guide patients and caregivers as they navigate the complex world of primary and specialty care, screenings, treatment, and recovery. You will follow up with patients and their families to ensure their understanding of the care plan, resources within their community, access to care, medication management and home care needs. Utilize your nursing expertise to serve each patient’s unique needs, while ensuring quality and continuity of care. You will form strong and enduring bonds with patients, families and multidisciplinary colleagues. This is a full-time hybrid position, Monday-Friday 9:00am-5PM.

Requirements

  • Bachelor's degree in Nursing (BSN).
  • Current NYS Registered Nurse (RN) licensure.
  • 5 plus years of health care experience.
  • Flexibility and ability to work in a fast-paced, changing environment.

Nice To Haves

  • Two years case management in an inpatient, HMO, Insurance or other outpatient/ambulatory setting.
  • Master's Degree in nursing or healthcare related field.
  • Bi-lingual – English/Spanish.
