The Nurse Care Coordinator is a vital role within the DOW Midland Health Center, focusing on enhancing patient care through education and coordination of services. This position is designed to ensure that patients receive comprehensive support both inside and outside the health center, particularly for chronic conditions such as diabetes, asthma, and hypertension. The Nurse Care Coordinator will be responsible for tracking and maintaining accurate documentation of the coordination and referral processes in the patient's medical record, ensuring that all care provided is effective and meets regulatory guidelines. In this role, the Nurse Care Coordinator will implement the Patient Centered Medical Home Model philosophy, which emphasizes a holistic approach to patient care. This includes conducting patient education, managing referrals, and providing follow-up to ensure that patients are receiving the necessary care. The coordinator will also be responsible for electronic medical record (EMR) tracking and documentation, ensuring that all activities related to referrals are documented in a timely manner. Additionally, the coordinator will run and monitor referral reports, providing updates to providers and clinical staff as needed. The Nurse Care Coordinator will act as a resource for patients and internal partners, assisting with scheduling appointments and referring patients to appropriate resources. They will facilitate communication between service providers and patients, ensuring that all necessary information is exchanged and that patients receive timely follow-up on their care. This role also includes conducting staff education and coaching to keep the team informed about available program resources and updates. Furthermore, the Nurse Care Coordinator will engage in proactive outreach to improve patient health outcomes and assist in the planning of future programs.
