What you will do as an RN Care Coordinator: Coordinates care across the health care continuum within designated sub-specialty population(s) Serves as a single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services Completes pre-visit planning and post-visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visits Facilitates proactive disease management and preventative health care using evidence-based practice Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed. Performs other duties as determined by manager and clinic needs

Requirements

  • Current Minnesota RN license
  • BLS certification
  • 3+ years of clinical nursing experience.
  • Demonstrated experience in patient advocacy and education, coaching, and proactive disease management
  • EMR and keyboarding/computer skill
