Hospital at Home is seeking a self-driven and detail-oriented team member who excels in multidisciplinary relationship building and thrives in a fast-paced environment. This role includes building strong professional relationships and collaborating effectively with healthcare providers and interdisciplinary team members to identify suitable patients for the program.

Requirements

  • Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program.
  • Requires Basic Life Support certification through the AHA.
  • Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
  • May require specialty certification.
  • 2+ years of inpatient acute nursing care experience preferred.
  • 1+ years of Case Management experience preferred.

Responsibilities

  • Building strong professional relationships and collaborating effectively with healthcare providers and interdisciplinary team members to identify suitable patients for the program.
  • Educating patients and their caregivers about the Hospital at Home model, ensuring a smooth transition and understanding of their care plan.
  • Providing nursing, administrative, and operational guidance to program staff.
  • Assisting with operational and business functions, managing projects, and acting as a liaison to other departments.
  • Serving as a clinical expert, coordinating, and evaluating the delivery of evidence-based care aligned with national and statewide clinical guidelines.
  • Facilitating access to care, leading performance improvement initiatives, and supporting development of clinical pathways.
  • Ensuring cost-effective, safe, and high-quality patient care while enhancing the overall patient experience.
  • Maintaining adherence to program standards and quality metrics established by the Service Line.
