About The Position

Saint Luke's North is looking for a Care Coordinator to join the team! This role is ideal for a RN looking to make a difference while working in a team setting. Shift: 8:00am - 4:30pm - M-F with occasional weekend coverage & holiday coverage. Location: Saint Luke's North - 5830 NW Barry Rd Required: MO RN license & Completion of BSN degree Responsible for ensuring quality patient outcomes and overcoming obstacles to delivery of care through interdisciplinary coordination and communication. This position coordinates the interdisciplinary team and partners with physicians to ensure patient treatment plan progress. The Care Coordinator will ensure that an appropriate plan of care is initiated and will monitor care progression to ensure quality outcomes and effective transitions of care. Requirements

• Applicable Experience: Less than 1 year
• Bachelor's Degree - Nursing
• MO RN license
• Completion of BSN degree
