As an RN Care Coordinator, you will work closely with a highly engaged interdisciplinary urogynecology team to support patients with complex and specialty care needs across the continuum of care. This role serves as a primary point of contact for patients, conducting clinical assessments, coordinating care plans, and providing education and outreach to improve outcomes and patient satisfaction. Responsibilities include managing prior authorizations and appeals for procedures, ensuring accurate order placement, performing chart reviews, coordinating surgical clearances, optimizing provider schedules, preparing patients for procedures, triaging clinic patients, and facilitating home care services as needed. You will collaborate with providers, insurance teams, and support services to close care gaps, improve access, and reduce avoidable delays while maintaining a strong focus on patient-centered, high-quality care. A caregiver in this role will primarily work 8:00am to 4:30pm. This is a hybrid role. For working days in the office, there is dedicated space at Hillcrest Hospitals. Office space can be arranged if a west location is preferred. A caregiver who excels in this role will: Work collaboratively with a multidisciplinary care team across the continuum of care for high-risk patients to develop goals, plan interventions and maximize patient outcomes. Provide care and disease management coordination. Identify patients in the specialty care practice that have ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments that include disease/age-specific, medical, behavioral, pharmacy, social and end of life needs of each patient. Inform and work with patients and their families regarding coordination of their care, provide education and coaching, monitor patient compliance with their care plan, perform reassessments regarding patient progress toward goals, and update plan of care. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across care settings, ensuring optimal communication and planning. Identify barriers, facilitate solutions, and connect others to community resources.
