Their hard work, dedication, and commitment has led to a decrease in Emergency Department visits, observation status, inpatient stays and hospital readmission in care coordinated patients. A caregiver in this position works a hybrid schedule from 8:00AM - 5:00PM. 50% work from home/50% in the office.
