About The Position

Taussig RN Care Coordinators care for patients as the one point of contact throughout their oncology journey. They provide education to patients and their families regarding their disease, treatment plans, medications, and side effects and utilize their nursing knowledge and critical thinking skills to triage symptoms as they arise, responding to patient phone calls and MyChart messages. This nursing position is truly the best of both worlds as you are still patient-facing, patient-focused and a patient-advocate without the stress and strain of bedside shift work, working weekends or holidays. You can leave work each day knowing the impact of your role during the vulnerable time in our patient’s lives. After six months of employment, you will have the opportunity to enjoy the flexibility of working from home one day per week, while remaining connected and collaborative with your team onsite for the remainder of the week.

Requirements

  • Graduate from an accredited school of professional nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) through American Heart Association (AHA)
  • Three to five years of RN experience required
  • Any registered nurse or advanced practice nurse must obtain a cancer specific certification or demonstrate ongoing qualifying education within the timeframe of the facilities accreditation cycle, if they work in medical oncology, radiation oncology, cancer center or cancer clinic and/or administer chemotherapy within an accredited Cleveland Clinic facility

Nice To Haves

  • Bachelor of science in nursing (BSN)
  • RN experience in direct patient care, care coordination and/or oncology
  • Specialty certification

Responsibilities

  • Work collaboratively with a multidisciplinary team across the continuum of care
  • Provide education and coordination of care regarding disease, treatment plan and medications
  • Serve as a liaison and advocate for patients and families
  • Assist in managing transitions of care across care settings, ensuring optimal communication and planning
  • Identify barriers, facilitate solutions and connect others to internal and community resources
