We are seeking an RN Care Coordinator in our Dermatology Department located in Durham, NC. The RN Care Coordinator (RCC) is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. The Dermatology RCC assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high risk patients or those with complex care needs (including multidisciplinary care). The RCC has a particular focus on improving transitions in care from Teledermatology to in-person appointments via e-comms and other urgent referrals. Additionally, the RCC functions as a clinical advocate and educator for patients, assisting with complex medication support through prior authorizations and patient assistance programs. The RCC will advance the mission of ensuring the right patient has access to the right provider at the right time, and will work with the edit to refine our decision tree and scheduling algorithms to promote access. The RCC will build community relationships with Dermatology's Clinically Integrated Network to provide expedient and reliable access to quality care. The RCC identifies and alleviates stressors during transitions and removes barriers to care with the aim to deliver a seamless model of care that benefits patients, family members, providers, and the healthcare team.
