Requirements

  • Missouri Registered Nurse license
  • Associate's Degree in Nursing, required; Bachelor's preferred
  • Minimum 3 years experience as registered nurse, 5 years experience preferred
  • Current Basic Life Support certification

Responsibilities

  • Identifies and actively manages a panel of high acuity patients in conjunction with physicians and care teams.
  • Assesses patients and provides effective, efficient medical care while managing the costs of treatment.
  • Works with physician leadership to define quality measure and outcome reporting processes and develop protocols and point of care reminders using nationally recognized EBC measures and outcomes.
  • Helps the on-site clinical care teams provide follow-up contact with patients to ensure compliance with recommendations, referrals to specialists, hospitalizations, ER visits, ancillary testing, and other services.
  • Works with consulting physicians, hospitals, ER, and other healthcare resources to clarify roles and develop effective and timely communication between PCP and these entities.
  • Effectively uses registries and other population management tools provided by payers. Oversees reporting to ensure adherence.
