Requirements

• Missouri Registered Nurse license
• Associate's Degree in Nursing (required); Bachelor's preferred
• Minimum 3 years experience as a registered nurse; 5 years experience preferred
• Current Basic Life Support certification

Responsibilities

• Identify and actively manage a panel of high acuity patients in conjunction with physicians and care teams.
• Assess patients and provide effective, efficient medical care while managing treatment costs.
• Work with physician leadership to define quality measures and outcome reporting processes.
• Develop protocols and point of care reminders using nationally recognized EBC measures and outcomes.
• Assist on-site clinical care teams with follow-up contact with patients to ensure compliance with recommendations and referrals.
• Collaborate with consulting physicians, hospitals, ER, and other healthcare resources to clarify roles and improve communication.
• Effectively use registries and other population management tools provided by payers.
• Oversee reporting to ensure adherence to quality measures.
