Requirements

  • Bachelors Of Science Nursing and One year experience in lifestyle management, exercise training, prescription and supervision of medical patients., upon hire
  • One year experience providing nurse case management, leading group exercise and education classes and Registered Nurse: CA, upon hire
  • Basic Life Support - CPR, upon hire
  • Advanced Cardiac Life Support, upon hire

Nice To Haves

  • Masters Of Science or advanced degree in nursing or related field
  • Two years experience in lifestyle management, exercise training, prescription and supervision of medical patients. , upon hire
  • Two years experience providing nurse case management, leading group exercise and education classes.
  • Certified Diabetes Educator, upon hire

Responsibilities

  • Clinical Practice & Case Management- Provides safe therapeutic care in a holistic and systematic way.
  • Incorporates differences into the provision of care.
  • Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education.
  • Responsible for developing, overseeing and implementing comprehensive case management and treatment plans for patients with cardiac and pulmonary diseases.
  • Utilizes the AIDET (Standard Patient Satisfaction Criteria) to communicate with others, with a focus on patients and their families.
  • Provides monitored exercise therapy to clients in the Lifestyle Management Program (i.e. with the diagnosis of heart disease, COPD, diabetes, PAD, obesity, etc.).
  • Maintains current clinical data, insurance information and program records on each client.
  • Adjusts treatment plan in accordance with established protocols, co-morbidities, client goals/needs and medical history.
