About the position
The position is within the Cardiac Rehabilitation department at Seton Edgar B Davis Hospital in Luling, Texas. The role involves implementing and monitoring patient care plans, serving as a primary coordinator for patient care, and assessing discharge planning needs with the healthcare team. The schedule is a 12-hour day shift.
Responsibilities
- Implement and monitor patient care plans.
- Monitor, record and communicate patient condition as appropriate.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Note and carry out physician and nursing orders.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.
Requirements
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
- Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.
