About The Position

The RN Cancer Center Navigator plays a crucial role in guiding cancer patients through the healthcare system, serving as an advocate and consistent contact for patients and their families. This position requires a compassionate individual who can effectively communicate and collaborate with healthcare providers, ensuring timely coordination of care and support throughout the patient's journey. The Navigator will also monitor quality outcomes and develop necessary tools and resources to meet the needs of cancer patients, while adhering to the mission and values of Blessing Health System.

Requirements

• Graduate of an RN School of Nursing.
• Excellent written and verbal communication skills.
• 3-5 years of experience in a cancer-related field.
• Possesses current State RN license.

Nice To Haves

• BSN degree.
• Experience in oncology and chemotherapy nursing.
• Experience in developing, implementing, and evaluating clinical outcomes.

Responsibilities

• Guide cancer patients through the healthcare system as an advocate and contact person.
• Provide support and education to patients and families.
• Coordinate care throughout the patient's experience.
• Build relationships with primary care providers and departments such as Surgery, Oncology, and Radiation Therapy.
• Identify barriers and connect patients with needed support services.
• Participate in multi-team committee activities and monitor quality outcomes.
• Produce audit and tracking information related to patient care.
• Develop necessary tools and resources for cancer patients.
• Assist with thoracic, colorectal, and oral cancer navigation duties.
