Pay Range: $43.90 - $65.86 Scheduled Weekly Hours: 40 Position Overview SUMMARY (BASIC PURPOSE OF THE JOB) Provides individualized education, psychosocial support and exercise training to patients receiving cardiac or pulmonary rehabilitation in collaboration with the medical directors and the multidisciplinary team. Uses nursing process to assess, plan, implement and evaluate patients. Assesses educational needs and provides disease specific education. Maintains a safe and clean environment for patients. MINIMUM REQUIREMENTS Education: Bachelor's degree in nursing or Associates degree in nursing with current RN experience. Graduation from an accredited school of nursing and enrolled in a BSN program may be considered. Experience: Two years critical care experience or training including cardiopulmonary assessment, and advanced dysrhythmia recognition. Other Credentials: AHA BLS - Healthcare Provider,Registered Nurse - NJ Requires ATCN or TNCC (or must obtain within 1 year of hire) AND 8 hours of Trauma related Continuing Nursing Education (CNE) contact hours annually if assigned to: RMC ICU/CCU, RMC Surgical Trauma Unit, RMC PACU, Emergency Department (all campuses) Requires NIH Stroke Scale Certification (or must obtain by end of orientation) AND 8 hours of Stroke related Continuing Nursing Education (CNE) contact hours annually if assigned to: Critical Care, Intermediate Care Unit, Emergency Department, Neuro Units, Cardiology Inpatient at Hopewell, Peds ED, PACU, Interventional Radiology, CNI, Observation CPR Requirements: Requires ACLS (or must obtain within 6 months of hire date) if assigned to: Critical Care/Intermediate/Telemetry, Emergency Rooms, Pediatrics/Pediatrics Emergency Room, Labor & Delivery, Surgical Services (not to include Perioperative), Interventional Procedures, Observation. Requires NRP (or must obtain within 6 months of hire date) if assigned to: Maternity Services, Emergency Room RMC/Deborah Requires PALS (or must obtain within 6 months of hire date) if assigned to: Emergency Rooms, Infant Follow-Up, Surgical Services (only PACU & Same Day Surgery), Pediatrics/Pediatric ED. Knowledge and Skills: Performs independently; excellent organizational, verbal and written communication skills; excellent customer service. Special Training: Strong background in cardiac and pulmonary assessment. ESSENTIAL FUNCTIONS Provides individualized education, psychosocial support and exercise training to patients receiving cardiac or pulmonary rehabilitation in collaboration with the multidisciplinary team and the medical directors of each program. Assesses educational needs. Performs initial admission interview, medical record review, risk factor identification, and program orientation as part of the patient's enrollment into the outpatient program. Incorporates the nursing assessment into the plan of care. Establishes goals of therapy with patient/family and multidisciplinary team. Documents a progress report midway and upon program completion. Provides a final report to physician that summarizes patient's goals/outcomes. Formulates and documents a final report to patient that evaluates individualized goals and establishes a discharge exercise prescription that maximizes the patient’s ability to continue with meaningful life activities. Educates cardiac and pulmonary inpatients(Phase I) as to their disease process. Reviews the outpatient cardiopulmonary rehabilitation program. Coordinates timely entrance into outpatient rehabilitation, if appropriate. Provides education information and offers coping mechanisms at the level of patient comprehension. Facilitates communication/evaluation of education. Develops individualized exercise prescriptions and makes appropriate adjustments to the treatment plan. Identifies possible patient problems and accurately documents clinical changes, interventions and outcomes. Communicates to the patient's physician regarding patients issues/problems that may arise during the session/program and completes documentation. Develops an individualized teaching plan and incorporates aspects of cultural diversity specific to the patient. Provides and documents formal and informal individualized education to the level of the patient. Confers with the multidisciplinary team regarding special needs of patient and family. Identifies situations that require immediate intervention and promptly alert nursing staff/supervising physician and or follows CHS emergency protocol. Implements delegated emergency measures safely, rapidly and accurately. Recognizes changes in patient condition and seeks appropriate assistance. Follows orders accurately and safely. Ensures hand off communication which includes a report of the situation. Uses discretionary judgment to appropriately modify patient care regimens in collaboration with the staff. Evaluates the quality of cardiopulmonary services, identifies problems, and initiates performance improvement activities. Documents a continuum of care that addresses outcome goals specific to the patient (Behavioral, Nutrition, Exercise, Education). Completes competencies that demonstrate proficiency of technical skills specific to the department. Follows policy and procedures specific to Cardiopulmonary Rehabilitation and participates in revisions as necessary. Participates in the evaluation of unit standards of care and identification of actual or potential problems. Organizes unit activities with multidisciplinary team to mobilize resources in a changing environment. Follows established policies and procedures for attendance, punctuality and continuing education. Responds to the changing needs of the department and has the ability to provide assistance to other team members. Displays flexibility in scheduling based on department needs and is self-motivated to develop a plan for adjusting staffing patterns. Attends continuing education programs and disseminates information to colleagues. Participates as a team member to plan and implement specific goals/activities. Assists with Performance Improvement (PI) activities (Plan, Do, Check, Act) and completes documentation process. Participates on hospital based committees. Provides employee/corporate/community based lectures/activities as required. Attends professional meetings related to Cardiopulmonary rehabilitation. Explores opportunities to market the Cardiopulmonary rehabilitation Program and gain referrals. Performs other duties as assigned and possesses a willingness to adapt to changing department demands.
