About The Position

The RN Baylor provides skilled nursing care to hospice patients during weekends and one weekday, ensuring comprehensive assessment, symptom management, treatment, and coordination of care. The nurse collaborates with patients, families, caregivers, and interdisciplinary teams to support end-of-life care needs. The role includes emotional support, patient education, and compliance with care protocols in a variety of care settings. Work hours include every Saturday and Sunday from 8am-8pm and one weekday (either Monday or Friday) from 830a-5p, based on organizational needs.

Requirements

  • Strong assessment and symptom management skills
  • Effective communication and emotional intelligence
  • Proficiency in documentation and use of EMRs
  • Ability to function effectively in emergencies and independently solve problems
  • Familiarity with hospice care principles and interdisciplinary collaboration
  • Minimum of two (2) years of experience in acute care, oncology, home health, or hospice settings preferred
  • Current Registered Nurse license in the State of Florida
  • Current CPR certification
  • Valid Florida driver’s license and reliable transportation with an acceptable driving record
  • Degree from a diploma nursing program, associate degree, or bachelor’s degree in nursing
  • Medical clearance for communicable diseases before patient contact
  • Current immunizations, including but not limited to TB, Hepatitis, and Flu
  • Participation in internal training and continuing education programs
  • Adherence to the American Nurses Association professional standards and the Nurse Practice Act

Responsibilities

  • Perform skilled nursing assessments and interventions in accordance with care plans and physician orders.
  • Provide education to patients and caregivers on disease progression and end-of-life expectations.
  • Collaborate with patients, caregivers, and healthcare practitioners to manage symptoms and adjust care plans.
  • Maintain real-time communication with supervisors and clinical teams, reporting patient status changes and visit outcomes.
  • Document all care encounters in the electronic medical record (EMR) in a timely and accurate manner.
  • Participate in interdisciplinary group (IDG) meetings and provide end-of-shift reports as needed.
  • Maintain professional standards of nursing, adhere to ethical conduct, and pursue ongoing education.
  • Participate in agency quality assurance activities and comply with compliance protocols.
  • Perform other related duties as assigned.
