About The Position

Liberty Healthcare Corporation is seeking Registered Nurses for Quality Reviewer positions that provide technical assistance to Service Coordinators conducting assessments using the interRAI-HC tool. This hybrid role involves both fieldwork and remote work, with a focus on ensuring best practices in assessments and service plans.

Requirements

• Registered Nurse with experience in conducting functional assessments of older adults and adults living with disabilities
• Experience in Home and Community Based Services (HCBS) or Long Term Services and Supports (LTSS)
• Experience in case management and service coordination
• Experience in leadership, management, supervision, or consulting
• Experience in providing technical assistance and training
• Subject matter expertise with the interRAI-HC tool is strongly preferred

Responsibilities

• In-person and virtual training and technical assistance with Service Coordinators
• Applying subject matter expertise with the interRAI-HC tool
• Ensuring best practices are used in the administration of the interRAI-HC tool
• Conducting quality reviews of assessments and service plans
• Proficiency testing
• Developing and implementing performance improvement plans and monitoring efficacy
• Collaborating with supervisors, managers, trainers, and educators
• Data analysis and reporting
