About The Position

King David Center for Nursing and Rehabilitation is looking for an RN Assessment Coordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY! Who We Are The Allure Group is a consortium of six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post-acute and sub-acute skilled nursing and rehabilitation. Requirements

Valid, current RN license
Previous experience in long-term care or skilled nursing facility
One-year job-related experience in the MDS process
Familiarity with various computer software
COVID-19 Vaccination

Responsibilities

Conduct and coordinate the completion of MDS assessments
Maintain and update all care plans and assessments
Examine and review residents’ medical history
Inform other nursing staff personnel of any persistent issues
Participate in the admission process while assessing prospective residents’ nursing needs and appropriate placement
Coordinate case management process
Ensure that the facility adheres to all regulatory requirements and standards of practice in relation to the MDS care planning process
Cooperate with inter-departmental personnel to ensure services can be adequately maintained to meet the needs of the residents
