About The Position

The position involves planning and prioritizing care for individuals and populations, focusing on strategies to promote optimal health. Requirements

  • Bachelor's Degree or equivalent in nursing or a healthcare-related field.
  • 2 years of relevant experience in clinical nursing or case management/managed care.
  • 5 years of relevant experience in clinical nursing or case management/managed care (preferred).
  • MI Registered Nurse (RN) license (preferred).
  • Basic Life Support (BLS) certification from AHA or ARC required within 90 days.
  • Case Manager, Certified (CCM) certification preferred upon hire.

Responsibilities

  • Plan and prioritize care for individuals and populations.
  • Demonstrate expertise in nursing care and manage a caseload of patients of varying complexity.
  • Improve patient, family, and health systems/community outcomes through educational concepts and preventive care.
  • Ensure continuous and timely availability of urgent care, emergency, inpatient, and operative services.
  • Assess internal and external referrals to identify patient needs and resources.
  • Develop care plans and make recommendations to healthcare providers regarding care management strategies.
  • Assess educational needs of patients and develop appropriate teaching strategies.
  • Communicate and collaborate with patients, providers, and payers to coordinate services.
  • Measure patient care outcomes and analyze data trends for groups of patients.
  • Document patient data, plans, interventions, and outcomes according to guidelines.
