About The Position

For more than 30 years, Adventist Health Vallejo has been helping people of all ages reach their fullest potential. We are comprised of a freestanding 61-bed facility with short-term psychiatric care for children, adolescents and adults. We also provide partial hospitalization programs for adults. Job Summary: Ensures efficient screening, assessment and coordination of both internal and external referrals. Maintains patient records and collaborates with physicians and hospital program staff, families, social network, and community agencies, if appropriate, to determine disposition of the patient. Functions as subject matter expert in admissions.

Requirements

  • Bachelor's Degree in Nursing (BSN): Preferred
  • Registered Nurse (RN) licensure in the state of practice: Required
  • Techniques for Effective Aggression Management (TEAM) or Crisis Prevention Intervention (CPI): Required

Responsibilities

  • Conducts initial health assessments, which identifies clinical needs, problems and concerns for safety, emotional, physical and financial parameters. Obtains initial insurance and demographic information and consults with admitting staff to determine viability of insurance. Obtains, when possible, pre-certification of patient admissions from health plan or managed care agents. Collects information regarding clients' presentation through a combination of interviewing, observation, tests, medical record review and consultation with community providers.
  • Facilitates appropriate admission arrangements. Provides appropriate referrals to community resources when admission to program or service is not indicated. Maintains current knowledge of programs and other available resources for all patient populations.
  • Completes and maintains client-related paperwork regarding patient rights, advanced directive protocols, confidentiality, and abuse reports. Maintains an understanding of regulations regarding mental health laws, Medicaid and Medicare, and other funding sources. Evaluates, and collects statistics relating to assessments, patient needs, plans, interventions, and outcomes.
  • Provides crisis intervention to achieve stabilization for the patient with the goal of achieving the least restrictive level of care.
  • Performs other job-related duties as assigned.
