The Admissions Registered Nurse (RN) in accordance with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families, in a collaborative relationship, with other members of the interdisciplinary team. The Admission Nurse makes initial nursing assessments, judgments and treatment decisions based on patient/family needs and wishes in accordance with professional skills.

Requirements

  • At least two years of public health/community health, med surg or related nursing experience.
  • Licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia.
  • Healthcare Provider BLS (CPR) certification.

Nice To Haves

  • Experience in home care, hospice, and/or oncology nursing.
  • BSN preferred.

Responsibilities

  • Supervise LPNs, CNAs, Unit Secretaries, and Volunteers.
  • Initiate appropriate referrals to other disciplines and/or community.
  • Implement approved emergency medications appropriately and effectively.
  • Initiate preventive and rehabilitative nursing procedures to maximize quality of life.
  • Function as a member of the regional and admissions team by demonstrating flexibility in work schedules and shared responsibility for delivery of care.
  • Conduct orientation, teaching, mentoring, and training or supervision of the orientation and training of Hospice employees and volunteers assigned to the clinical service.
  • Participate in quality improvement activities, QUAPI program and hospice sponsored inservice training.
  • Serve on committees or teams as approved by the clinical manager.
  • Assume responsibility for own professional growth and development.
  • Explain hospice philosophy, goals, and services to patients and families.
  • Secure admission to Capital Caring Health in collaboration with Team Social Worker.
  • Choose appropriate Hospice program to best meet patient/family needs.
  • Initiate the patient's plan of care in accordance with nursing needs specified in the approved plan of treatment.
  • Document care provided into the medical record in a timely manner.
  • Teach and supervise the teaching of family/household members/persons responsible for assisting the patient.
  • Deliver or supervise the delivery of skilled nursing services as outlined in the plan of care.
