Role Snapshot Position: RN Admissions Nurse – Hospice (PRN) Focus: Hospice admissions, eligibility assessment, and initiation of care Schedule: PRN Work Setting: Patient homes, hospitals, and care facilities Coverage: Assigned service area within the posted location Role Summary This per diem role offers experienced hospice nurses the opportunity to support patients and families at the very start of their care journey. As a RN Admissions Nurse – Hospice (PRN), you will provide clinical presence, clear guidance, and compassionate reassurance during admission visits, helping ensure a smooth and timely transition into hospice services. This role offers flexibility while remaining closely connected to a collaborative clinical team. Overview The RN Admissions Nurse is responsible for completing clinical evaluations, admission visits, and required documentation for patients referred to hospice services. This role ensures timely initiation of care, compassionate communication with patients and families, and close collaboration with intake, admissions, and clinical teams. All services are delivered in accordance with hospice regulations, Medicare Conditions of Participation, and organizational standards.

Requirements

  • Graduate of an accredited nursing program; Associate’s or Bachelor’s degree preferred
  • Current RN license in the state of practice or valid multistate compact license with ability to practice in all states of operation
  • Minimum of one (1) year of hospice, palliative care, home health, or hospital-based nursing experience preferred
  • Strong clinical assessment, communication, critical thinking, and documentation skills
  • Ability to compassionately educate and guide patients and families through emotionally sensitive conversations
  • Familiarity with hospice eligibility requirements, services, and Medicare Conditions of Participation
  • Valid driver’s license and reliable transportation required

Responsibilities

  • Conduct clinical evaluations of patients referred for hospice, assessing eligibility and appropriateness per Medicare Conditions of Participation and organizational guidelines
  • Meet with patients and families to explain hospice services, review consents, answer questions, and guide them through the admission process
  • Collaborate with intake, admissions, and clinical teams to ensure timely onboarding and initiation of hospice services
  • Coordinate with attending physicians, hospital discharge planners, and referring providers to obtain orders and ensure continuity of care
  • Complete and document admission visits, including initial assessments, medication reconciliation, and initiation of the hospice plan of care
  • Provide emotional support to patients and families, addressing concerns and setting expectations for hospice services
  • Ensure accurate and timely documentation to support clinical, regulatory, and billing compliance
  • Maintain current knowledge of hospice eligibility criteria, services, and regulations, serving as a resource to internal and external partners
  • Provide after-hours or on-call admission support as needed
  • Support the clinical team with routine or symptom-focused visits as needed
  • Perform other duties as assigned
