Requirements

  • Graduate of an accredited school of nursing.
  • Associates degree in nursing.
  • Registered as RN with New Hampshire’s Board of Nursing.
  • A minimum of one (1) year recent acute care experience.
  • Ability to exercise initiative and independent judgment.
  • Excellent verbal and written communication skills and competent in problem solving abilities.
  • CPR Certification.
  • Satisfactory criminal record.
  • Satisfactory motor vehicle record.

Nice To Haves

  • Bachelor’s degree in nursing.
  • Community Health/Education experience.
  • Certifications, which may include: IV; Chemotherapy; Ostomy.
  • Familiarity with OASIS and what drives reimbursement.
  • Allscripts’ EMR software proficiency.
  • Supervisory experience over home health aides.

Responsibilities

  • Admission to HHHC's homecare division.
  • Completes an initial home health assessment of patient and family to determine home care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Utilizes the nursing process in planning, providing, and evaluating skilled needs.
  • Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Includes the patient and family on the planning and revising process.
