About The Position

The Admissions RN at Pikes Peak Hospice & Palliative Care is responsible for providing assessment, supportive care, and education to prospective patients and families regarding hospice services. This full-time position involves determining hospice appropriateness, assisting in patient admissions, and acting as a liaison with hospitals, facilities, and physicians. The role requires effective communication and coordination to ensure a smooth transition into hospice care for patients and families.

Requirements

  • Graduate of an accredited school of nursing; BSN preferred.
  • One to two years of experience in hospice palliative and/or home health care, medical/surgical, public health, oncology, acute care, or home health.
  • Current Colorado RN licensure.
  • Current CPR certification.
  • Valid Colorado State driver's license and auto insurance.

  • Experience in hospice palliative care or home health care is preferred.

Responsibilities

  • Provides assessment, supportive care, and education to prospective patients and families regarding hospice services.
  • Determines hospice appropriateness of the patient and assists in admitting patients to the hospice program.
  • Acts as a liaison with assigned hospitals, facilities, and physicians.
  • Assesses and counsels patients and families during initial interviews to determine the level of care and psychosocial needs.
  • Accurately gathers information and data, reviews charts, and records information.
  • Provides education on hospice philosophy and completes all admission paperwork.
  • Initiates the clinical record through the EMR process, demonstrating knowledge of admission protocol.
  • Assesses and places orders for immediate medication needs and oversees placement orders for medical equipment and supplies.
  • Consults with hospital staff and discharge planners to facilitate patient transitions to hospice care.
  • Completes the full Comprehensive Assessment at admission and communicates pertinent information to the interdisciplinary team.
