As an RN Admission Specialist with Home Health Care, Inc., you’ll deliver expert OASIS documentation and launch personalized care plans for clients transitioning into home -based care. Requirements
- Minnesota RN license in good standing
- Admission and OASIS experience
- Strong communication and time management
- Reliable transportation
Responsibilities
- Perform comprehensive patient assessments and OASIS documentation
- Initiate individualized care plans
- Recommend and coordinate additional services
- Communicate with providers and caregivers to ensure safe transitions
