About The Position
Take charge of the start of care journey. As an RN Admission Specialist with Home Health Care, Inc., you’ll deliver expert OASIS documentation and launch personalized care plans for clients transitioning into home -based care. Requirements
- Minnesota RN license in good standing
- Admission and OASIS experience
- Strong communication and time management
- Reliable transportation
Responsibilities
- Perform comprehensive patient assessments and OASIS documentation
- Initiate individualized care plans
- Recommend and coordinate additional services
- Communicate with providers and caregivers to ensure safe transitions
