About The Position

Registered Nurse (RN) – Home Health Admissions Company: Mozaic Home Health Care Position Summary The Registered Nurse (RN) – Home Health Admissions is responsible for completing start-of-care (SOC) and admission visits in patients' homes. This role conducts comprehensive in-home assessments, completes required documentation (including OASIS, as applicable), and collaborates with the interdisciplinary team to ensure safe, compliant, and patient-centered home health services.

Requirements

  • Active Registered Nurse (RN) license in the State of Connecticut.
  • Minimum of 1 year of nursing experience; home health, admissions, or start-of-care experience preferred.
  • Current Connecticut driver's license with reliable transportation and ability to travel within assigned service area.
  • Knowledge of OASIS-E documentation and CMS home health regulations strongly preferred.
  • Familiarity with Connecticut Department of Public Health (DPH) regulations for home health agencies.
  • Strong clinical assessment, critical thinking, and time-management skills.
  • Excellent communication and interpersonal skills with patients, families, physicians, and interdisciplinary team members.
  • Ability to work independently in the field while collaborating with a multidisciplinary team.
  • Proficiency with electronic medical records (EMR) and mobile documentation systems.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.

Responsibilities

  • Complete home health admission and start-of-care visits in patients' homes.
  • Perform comprehensive nursing assessments, including physical, psychosocial, and environmental evaluations.
  • Accurately complete OASIS assessments and all required admission documentation in compliance with CMS and agency standards.
  • Collaborate with physicians, therapists, and interdisciplinary team members to develop individualized plans of care.
  • Educate patients and caregivers on diagnoses, medications, treatment plans, safety, and disease management.
  • Administer medications and treatments as ordered and within scope of practice.
  • Monitor patient status and promptly communicate changes in condition to the physician and care team.
  • Coordinate services, referrals, and follow-up visits to ensure continuity of care.
  • Adhere to infection control, safety, and documentation standards in the home setting.
