About The Position

The RN Acute Care Case Manager evaluates the patient's health status and the patient/family/significant other's care capabilities to facilitate the discharge planning process. The case manager develops a multidisciplinary, culturally appropriate, age-specific discharge plan. They also complete clinical reviews utilizing Internal criteria for the appropriate level of care and insurance coverage.

Requirements

• ADN or BSN required
• 3 years of case management-related health care experience.

Nice To Haves

• BSN preferred but not required
• Experience with patient documentation in an EMR system preferred

Responsibilities

• Assesses and identifies patients' needs during the discharge planning process
• Provide resources and community referrals for continuity of care
• Care coordination with families, providers, and clinical team
• Utilization Reviews
