The RN Tracheostomy School Nurse assumes responsibility and accountability for the application of the nursing process and the delivery of student/client care and/or contracted services in a school setting. This role is critical in ensuring that students with tracheostomies receive the necessary medical care in accordance with Amergis Healthcare's philosophy, standards, policies, and physician orders. The nurse will utilize the nursing process to plan, implement, and evaluate the care needs of students, demonstrating skillful nursing care in accordance with prescribed orders and Individualized Health Plans (IHP). In this position, the RN Tracheostomy School Nurse will perform various interventions, treatments, and medication administration according to the identified priorities outlined in the IHP. Monitoring and documenting the effectiveness of these interventions is essential, as is completing documentation per client expectations. The nurse will coordinate and facilitate services as requested between the school, local health agencies, and other community resources to meet the needs of the student. Effective communication with school district personnel regarding the student's plan of care is vital, as is notifying the District School Nurse, Amergis Clinical Designee, parents, or physicians about any changes in the student's condition. The role also requires collaboration with school and education personnel, as well as other health agencies, to design and meet the healthcare needs of students. The RN must be prepared to perform emergency trach changes if required by the student's respiratory status and provide trach care to maintain the patency of the tube while minimizing the risk of infection. Additionally, the nurse will evaluate and assure student safety at all times, adhere to Amergis Healthcare and school district policies, and maintain accuracy in student health records. Participation in student care conferences and Individualized Education Programs (IEP) may also be required at the request of the school district.
