As an LPN on our team, you’ll be a trusted partner to clients and families, helping them live comfortably, safely, and with dignity. You’ll work alongside supportive colleagues who are passionate about making a positive impact in our communities. What You’ll Do As an LPN, you’ll provide skilled nursing care that follows each client’s personalized plan of care. Under the supervision of a Registered Nurse, you will:

Requirements

  • Graduate of an accredited or state‑approved practical nursing program
  • Current LPN license in the state of practice
  • At least 1 year of experience in an acute care setting
  • Strong interpersonal skills and comfort working with clients, families, and interdisciplinary teams
  • Successful completion of NAPNES (or equivalent) medication exam prior to administering meds
  • A valid driver’s license and reliable transportation
  • Ability to consistently demonstrate Trinity Health Core Competencies
  • Ability to lift up to 35 lbs and assist clients with mobility as needed
  • Frequent lifting, standing, and walking
  • Occasional bending, stooping, twisting, and patient positioning
  • Must adapt to changing work needs and schedules
  • Travel to various Trinity Health worksites as required
  • Maintain a safe environment for clients, visitors, and staff

Nice To Haves

  • Home health experience preferred but not required

Responsibilities

  • Deliver high‑quality nursing care in alignment with physician orders and the RN’s direction.
  • Administer medications and perform treatments within your state’s LPN scope of practice.
  • Use your nursing knowledge to observe, assess, and respond to changes in client condition.
  • Teach and support family members to help them care for their loved one safely.
  • Communicate changes in client condition promptly to the Case Manager and physician.
  • Take part in care conferences and collaborate with the full care team.
  • Maintain clear, respectful communication with clients, families, and colleagues.
  • Work with the scheduling team to ensure your visits are timely and accurate.
  • Accurately document all nursing observations, actions, and evaluations the same day you provide care.
  • Submit electronic documentation on time.
  • Stay current on agency policies, ethical standards, and all Federal, State, and local regulations.
  • Incorporate best practices and continuous quality improvement into everyday work.
  • Demonstrate Trinity Health’s values and mission in all interactions.
  • Stay up to date in your area of practice and support excellence in client outcomes.
  • Work independently, responsibly, and with strong attention to detail—even with frequent interruptions or changing priorities.
