As an LPN on our team, you’ll be a trusted partner to clients and families, helping them live comfortably, safely, and with dignity. You’ll work alongside supportive colleagues who are passionate about making a positive impact in our communities. What You’ll Do As an LPN, you’ll provide skilled nursing care that follows each client’s personalized plan of care. Under the supervision of a Registered Nurse, you will:

Requirements

Graduate of an accredited or state‑approved practical nursing program
Current LPN license in the state of practice
At least 1 year of experience in an acute care setting
Strong interpersonal skills and comfort working with clients, families, and interdisciplinary teams
Successful completion of NAPNES (or equivalent) medication exam prior to administering meds
A valid driver’s license and reliable transportation
Ability to consistently demonstrate Trinity Health Core Competencies
Ability to lift up to 35 lbs and assist clients with mobility as needed
Frequent lifting, standing, and walking
Occasional bending, stooping, twisting, and patient positioning
Must adapt to changing work needs and schedules
Travel to various Trinity Health worksites as required
Maintain a safe environment for clients, visitors, and staff

Home health experience preferred but not required

Responsibilities

Deliver high‑quality nursing care in alignment with physician orders and the RN’s direction.
Administer medications and perform treatments within your state’s LPN scope of practice.
Use your nursing knowledge to observe, assess, and respond to changes in client condition.
Teach and support family members to help them care for their loved one safely.
Communicate changes in client condition promptly to the Case Manager and physician.
Take part in care conferences and collaborate with the full care team.
Maintain clear, respectful communication with clients, families, and colleagues.
Work with the scheduling team to ensure your visits are timely and accurate.
Accurately document all nursing observations, actions, and evaluations the same day you provide care.
Submit electronic documentation on time.
Stay current on agency policies, ethical standards, and all Federal, State, and local regulations.
Incorporate best practices and continuous quality improvement into everyday work.
Demonstrate Trinity Health’s values and mission in all interactions.
Stay up to date in your area of practice and support excellence in client outcomes.
Work independently, responsibly, and with strong attention to detail—even with frequent interruptions or changing priorities.
