About The Position

We are seeking a compassionate and proactive Retention Agent to join our team, specializing in retaining patients enrolled in our Remote Patient Monitoring (RPM) program for diabetes management. The Retention Agent will focus on engaging patients, ensuring adherence to their care plans, and providing educational support to improve health outcomes. This role is essential for fostering patient satisfaction, encouraging active participation in the program, and maintaining long-term patient relationships. In this position, you will proactively contact patients in the RPM diabetes program to check on their status and adherence to monitoring plans. You will provide motivation and encouragement to enhance patient engagement with health coaches and care plans, building and maintaining strong relationships with patients to foster trust and continued participation. Additionally, you will provide educational resources to help patients understand and manage their diabetes more effectively, conducting one-on-one or group sessions to reinforce the importance of adhering to the monitoring protocols within the care plan. You will also gather and analyze patient feedback to understand their needs, concerns, and satisfaction levels, using this feedback to enhance engagement strategies and improve the effectiveness of the RPM program. Collaboration is key in this role, as you will work closely with healthcare teams, including diabetes health coaches and clinical staff, to ensure patient care plans are communicated effectively and any issues are promptly addressed. Furthermore, you will maintain accurate and detailed records of all patient interactions and outcomes in patient management systems, providing regular reports on patient retention metrics, challenges, and successes to relevant stakeholders.
