About The Position

This position is located at Sixteenth Street Community Health Center in Milwaukee, WI. Pharmacy Hours: Monday-Friday 8:30am -6:00pm. What Health System Pharmacy contributes to Cardinal Health Pharmacy Operations is responsible for the safe, efficient and effective coordination of Cardinal Health's pharmacy operations that service acute care hospitals, hospital retail customers, ambulatory care and alternate site facilities, oncology and cardiology practices as well as retail customers. Retail Pharmacy is responsible for ensuring the correct and safe dispensing of prescription medications to the general public, in accordance with legal, ethical and professional guidelines; may also consult with and advise patients and physicians regarding prescribed and over-the-counter medications. Community Health Center – Community Pharmacy Manager Cardinal Health is looking for a highly motivated pharmacist interested in managing an entity-owned 340B community pharmacy located within a partner health center. Our pharmacists play an important role in delivering medication and pharmaceutical care in communities that are most in need. This includes immunizations, patient consultation, medication adherence monitoring, medication therapy management, and management of high-risk patients.

Requirements

  • Bachelor’s degree in pharmacy required, PharmD preferred.
  • Bilingual (Spanish, English) preferred.
  • 3 years pharmacy management experience, 4-5 years overall pharmacy experience.
  • Active Wisconsin Pharmacist license in good standing.
  • Immunization Certification or willing to become a certified immunizer within 90 days of hire.
  • Ability to work a flexible schedule.
  • Interpersonal relations and customer service skills.
  • Multitasking and time management skills.
  • 340B Experience preferred.

Responsibilities

  • Collaboration with administrative and physician leadership within partner Community Health Centers to implement pharmacist driven clinical and dispensing services to the patients served.
  • Develop and implement collaborative practice agreements to help drive patient medication compliance for high-risk patients and improve provider satisfaction.
  • Implement best practices and ensure department compliance with 340B rules and regulations.
  • Ensuring adequate staffing is present to fulfill the needs of the pharmacy or pharmacy department.
  • Maintaining accurate records of all prescription medication received and dispensed.
  • Ensure policies are in place regarding accurate dispensing and labeling of prescriptions and those policies are followed.
  • Ensure that security of the prescription area and its contents are maintained at all times.
  • Oversee the Pharmacy’s Continuous Quality Improvement Plan to minimize medication error rates and maximize patient safety.
  • Not allowing misbranded, deteriorated, adulterated, improperly stored or outdated drugs to be dispensed.
  • Maintaining the prescription area in an orderly and sanitary manner.
  • Operate as 'Pharmacist in Charge' of the pharmacy in accordance with Wisconsin Pharmacy law.
  • Maintaining that pharmacy and all pharmacy personnel comply with all Federal and State statutes, rules and regulations governing the practice of pharmacy.
  • Other duties as assigned.
