About The Position

This is a Retail Pharmacist position working at Corewell Health Retail Pharmacy - St. Joseph providing Discharge Medication Program services for patients discharging from Corewell Health Lakeland Hospitals - St. Joseph Hospital. Provide daily patient focused clinical pharmacy services to inpatients and outpatients of all age groups of Lakeland Regional Health System to include, but not limited to, medication dosing, chart review, patient education, floor rounds and physician intervention. Participate on committees and give special reports. Rotate through the distributive pharmacy as scheduled. Directly perform order entry in the pharmacy; Provide all of the functions as a staff pharmacist; Perform other duties as assigned. Provide patient and community education; Participate in the staff development education program; Provide clinical training to Pharm. D. students as assigned. Perform accurate and timely input of interventions and ADR’s. Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Requirements

  • Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy.
  • 2 years of relevant experience.
  • American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice.

  • Ideal candidate will possess a PharmD Degree and Retail Pharmacy experience.
