Requirements
- Active California CNA certification; RNA experience/training preferred.
- Comfort with PCC/POC documentation and restorative workflows.
- A coach’s mindset—positive energy, great communication, and the ability to motivate residents kindly and consistently.
- Reliability, teamwork, and an eye for safe, person‑centered care.

Responsibilities
- Lead restorative programs that make a daily difference—ambulation/walking with assistive devices, bed mobility, transfers, ADLs, ROM (AAROM/PROM), sit‑to‑stand practice, strengthening and exercise support, and restorative dining initiatives.
- Chart progress clearly in PCC/Point of Care and support the RNA Weekly Summary process with the licensed nurse attestation.
- Collaborate with Rehab & Nursing to initiate/modify restorative plans (OT/PT/ST referrals), activate tasks in POC, and keep care plans current.
- Join the interdisciplinary RNA meeting (at least monthly) to review programs, documentation, and outcomes—operating with RN oversight and a team-first approach.
- Spot changes early (pain, mobility, positioning, swallowing) and escalate appropriately to keep residents safe and progressing.
