About The Position

Located near the Western Addition and Fillmore District in San Francisco, Central Gardens provides personalized rehabilitation and nursing services. We are looking for a dedicated Restorative Nurse Assistant (RNA). You will work with our therapy and nursing departments to help residents improve or maintain their mobility, strength, and functional independence. Requirements

  • Certifications: Current CNA license and RNA training/certification.
  • Experience: 1 year of experience in a long-term care facility preferred.
  • Education: High School Diploma or equivalent.
  • Skills: Proficient in data entry (Point Click Care); ability to solve practical problems and read technical procedures.
  • Physical: Ability to stand/walk for long shifts and safely lift/transfer patients (frequently exceeding 50 lbs).

Responsibilities

  • Restorative Care: Lead exercises, range-of-motion activities, ambulation, and transfers as directed by the Rehab team.
  • Daily Support: Assist residents with dining, splinting, and activities of daily living (bathing, dressing, and hygiene).
  • Clinical Accuracy: Obtain/record resident weights and document all care accurately in Point Click Care (PCC).
  • Skin Integrity: Reposition residents regularly and apply pressure-relieving devices to ensure comfort and prevent skin breakdown.
  • Collaboration: Participate in RNA meetings to monitor programs and report any changes in resident condition to the RN Supervisor or Rehab Department.
