The Restorative Aide provides restorative nursing services to residents and patients under the direction and supervision of licensed nursing staff and/or rehabilitation professionals. The Restorative Aide supports residents in attaining and maintaining their highest practicable level of physical, mental, and psychosocial functioning, in accordance with the resident’s comprehensive care plan, therapy discharge plans, physician orders, and applicable federal and state regulations. Essential Job Functions/Responsibilities Restorative Nursing Care Implements restorative nursing programs for SNF residents as outlined in individualized care plans, therapy discharge summaries, and restorative program schedules. Assists residents with ambulation, transfers, positioning, bed mobility, range of motion (active and passive), strengthening exercises, splinting, and contracture prevention. Provides functional training related to activities of daily living (ADLs), including eating, grooming, dressing, toileting, and use of adaptive equipment. Encourages resident participation and independence to prevent decline following skilled therapy discharge. Observes and reports resident tolerance, progress, or changes in condition to licensed nursing staff or rehabilitation professionals. Documentation & Regulatory Compliance Documents restorative services accurately and timely in accordance with CMS requirements, MDS guidelines, and facility policy. Tracks resident participation, minutes, progress, and outcomes of restorative programs. Communicates resident status and progress to the interdisciplinary care team. Supports survey readiness by ensuring restorative programs are implemented and documented consistently and appropriately. Safety & Infection Prevention Ensures resident safety during restorative activities by following proper body mechanics, transfer techniques, and use of assistive devices. Adheres to infection prevention and control policies consistent with SNF standards, including hand hygiene and PPE usage. Reports accidents, incidents, injuries, or unsafe conditions per facility protocol. Interdisciplinary Collaboration Works collaboratively with nursing staff, therapy services, and the interdisciplinary team to support resident-centered goals. Reinforces therapy recommendations and restorative interventions as part of the SNF care continuum. Provides emotional support, encouragement, and education to residents and families regarding restorative goals. Additional Duties Maintains restorative equipment in safe and sanitary condition. Attends required in-services, training, and competency evaluations related to SNF care. Performs other duties as assigned consistent with the scope of practice.
