About The Position

SUMMARY: Under the direction of the Program Manager or designee, provides of variety of services to infants, children, adolescents and adults who have special needs. Service Providers work with consumers in their homes or in the community.

Requirements

  • High school diploma or GED coupled with one year experience meeting the personal care needs of persons with disabilities.
  • Must possess ability to read, write and understand English. Strong oral and written communication skills are a must. Must be able to understand and follow verbal and written instructions from supervisor.
  • Knowledge and ability to work with basic mathematical concepts.
  • Must have working knowledge of personal computers. (Highly desirable)
  • Must possess a valid CDL and have clean driving record as well as operate a vehicle in good working order for employee transportation. Must have proof of ongoing auto insurance on personal vehicle.
  • Is punctual and dependable
  • Provides excellent internal and external customer service
  • Must work in an ethical and professional manner
  • Has strong interpersonal skills that include responding well to supervision
  • Must be able to work collaboratively
  • Must be able to work independently
  • Must demonstrate respect and sensitivity to diversity
  • Must possess general knowledge of developmental and physical disabilities; individual/group instruction techniques, behavioral and medical knowledge associated with given disabilities such as seizure activity, practice of correct lifting, positioning and transferring techniques, behavior modification techniques, documentation procedures and have ability to remain calm in high stress situations
  • Must maintain First Aid/CPR certificates and be able to meet California Community Care Licensing personnel standards
  • Able to relate to and establish rapport with families, disabled consumer, and fellow employees

Responsibilities

  • Attend to the physical care needs of the disable individual including bathing, feeding, grooming and light housekeeping chores
  • Motivate and initiate age appropriate developmental learning activities
  • May provide care for siblings in the home or community as requested and approved by Program Manager
  • Assists with medications within UCP policy and procedures
  • Implements respite service plan
  • Generates, maintains and organizes required reports and service delivery data
  • Other duties as assigned
