Essential Duties: The TeleCare Team Respiratory Therapist is responsible for patients with respiratory and respiratory–related equipment and for assessment of patient / client need and the selection, instruction, and troubleshooting of appropriate equipment and / or service provided to meet those needs and maintaining appropriate documentation

Requirements

  • Experience as respiratory therapist.
  • Thorough knowledge and skill related to respiratory care field.

  • Knowledge of general medical terminology.
  • Knowledge, ability, and communication skills to conduct research interviews, including the collection of confidential medical and respiratory health histories.
  • Experience in data cleaning, data checking, and data management.
  • Excellent interpersonal skills necessary for maintaining study integrity and credibility with physicians, office staff, medical staff, and patients.
  • Ability to work independently under multiple deadlines, follow instructions, work cooperatively with other staff members and be flexible to work on multiple projects.
  • Associates Degree or Equivalent
  • California Respiratory Care Practitioner license (RCP) or Certified Respiratory Therapist (CRT), Valid Basic Cardiac Life Support (BCLS)
  • Valid State Issued Driver’s License
  • Experience: 3-5 Respiratory Therapist Experience

Responsibilities

  • Receives and documents the order for respiratory equipment and / or Chronic Care Programs from the customer service, patient / client, caregiver, physician and family member.
  • Responsible driving from office to patient home or hospital.
  • Enroll patients into the appropriate Chronic Care Program as well as assess and educate patients on disease process.
  • Ensures comprehensive documentation of each patient interaction and follow-up in the SCH charting platform while actively coordinating with the team on escalated cases.
  • Collects additional information and / or authorization if referral is from other than the physician
  • Involves the patient / client and / or family, caregiver in the process.
  • Instructs patients / clients in the proper use and care of respiratory equipment according to the written policies and procedures for that equipment.
  • Completes patient / client accounts paperwork including Plan of Care / Treatment and assessments.
  • Acts as liaison between the company and the patient/client, family, caregiver & physician.
  • Assumes full responsibility for their own actions, professional skills, and attitudes.
  • Follow all policies and procedures as outlined in JCAHO.
  • Follows up visits for respiratory patients and assists in continuity of care as needed and determines by Policy and Procedure.
  • Provides 24-hour on call service, seven days a week as scheduled.
  • Sets up respiratory and respiratory related equipment for patients.
  • Provides in services for marketing department and all other department as necessary.
  • Demonstrates timeliness, courtesy, sincerity and patience when dealing with patients/clients.
  • Supports the company by being loyal and informing the office of concerns and problems to be resolved.
  • Determine requirements for treatment, such as type, method and duration of therapy, precautions to be taken, and medication and dosages, compatible with physicians' orders.
  • Communicates with the patient’s / client’s physician on changes in patient’s condition and/or needs and carries out adjustment to the patient’s / client’s Plan of Care / Treatment.
  • Perform weekly roundtable discussions regarding all patients enrolled into Chronic Care Programs during Interdisciplinary Care Team meetings.
  • Monitor patient's physiological responses to therapy, such as vital signs, arterial blood gases, and blood chemistry changes, and consult with physician if adverse reactions occur.
  • Read prescription, measure arterial blood gases, and review patient information to assess patient condition.
  • Maintain charts that contain patients' pertinent identification and therapy information & confidentiality.
  • Effectively operates within continuously developing software systems, i.e Healthcall/Jotform.
  • Utilize technology to enable Telehealth visits with patients.
  • Perform other duties as assigned.
