Trellis, a nonprofit in Minneapolis, MN, is looking to hire a full-time (40 hours/week) Minnesota Aging Pathways Resource Specialist. Do you enjoy working with older adults? A DAY IN THE LIFE OF A RESOURCE SPECIALIST In this position, you will provide comprehensive, objective telephone and/or in-person support to consumers inquiring about long-term care services, housing options, caregiver support, Medicare counseling, and other public benefits. You will also assist consumers in assessing their or their care receiver's needs for services, making decisions about service options, assessing benefits, and connecting with formal and informal providers. We provide community organizations with funding, and older adults with information and connections to access the best resources to help them age well. By partnering with community and healthcare organizations, we strive to build age- and dementia-friendly networks that make daily life easier for older adults.
