Trellis, a nonprofit in Minneapolis, MN, is looking to hire a full-time (40 hours/week) Minnesota Aging Pathways Resource Specialist. Do you enjoy working with older adults? A DAY IN THE LIFE OF A RESOURCE SPECIALIST In this position, you will provide comprehensive, objective telephone and/or in-person support to consumers inquiring about long-term care services, housing options, caregiver support, Medicare counseling, and other public benefits. You will also assist consumers in assessing their or their care receiver's needs for services, making decisions about service options, assessing benefits, and connecting with formal and informal providers. Requirements

  • Candidates must have experience with client interviewing and conducting person-centered assessments
  • well-developed interpersonal communication, customer service and problem-solving skills
  • intervention and de-escalation skills
  • knowledge of community resources, public assistance programs and a basic understanding of Medicare
  • the ability to use professional judgement to work independently
  • excellent time management
  • computer proficiency
  • When working offsite, employees must meet specific internet criteria including demonstrated high speed connect ability and the ability to meet privacy and security standards with secure (password protected) internet and demonstrate successful performance.
